Required Athletic Forms

Students in grades 6–12 who are interested in participating in athletics should review the 2026–2027 athletic start dates and eligibility requirements as early as possible.

Before a student may tryouts for a team, attend a practice, or participate in any athletic activity, all required FHSAA Athletic Eligibility Forms must be fully completed, signed, dated, and uploaded to the student’s account through AthleticClearance.com.

Important Eligibility Requirement

Students will not be permitted to participate in any athletic activity until all required documentation has been submitted, reviewed, and the student has received athletic clearance.

A delay in completing or submitting required forms, medical documentation, or certificates may result in a delay in athletic eligibility and could prevent a student from participating in tryouts, practices, games, or competitions.

Plan Ahead!

Do not wait until the beginning of your sport’s season to complete your athletic forms.

Students and families are encouraged to begin the clearance process as soon as possible. Once each required document is completed, upload it immediately to AthleticClearance.com.

Completing your paperwork early helps ensure that your eligibility can be reviewed in a timely manner and prevents unnecessary delays when the season begins.

Your clearance must be complete before you can participate.

Athletic Clearance Requirements

  • For the 2026–2027 school year, Shelton Academy will start using the FHSAA Athletic Clearance  platform (https://athleticclearance.com) to collect and verify all required eligibility documentation  for student-athletes in grades 6–12. Visit the Athletic Clearance webpage for complete step-by-step instructions, required FHSAA forms, NFHS course requirements, EL2 Medical, ECG/EKG information, registration instructions, and frequently asked questions.  
  • View videos and upload completion certificates for FHSAA Concussion, FHSAA Heat Illness Prevention, and FHSAA Sudden Cardiac Arrest, yearly courses. This is a yearly requirement by the Florida High School Athletic Association and the State of Florida.  Please visit nfhslearn.com to take the required courses. All NFHS Learn courses are free.
  • Students may not participate until officially cleared. Paper forms, emailed documents, or physical copies submitted outside of Athletic Clearance will not be accepted.
  • International students are required to upload (I-94, Passport, and US Visa) documentation for the FHSAA Registration for Youth Exchange International Student (EL4) (will be processed by Shelton Academy Athletics). International students require a F1 or J1 Visa for athletic participation. Please send an email to Coach Julio Goncalves Coach Julio Goncalves, communicating that your son is an International Student.
  • Incoming transfer students and parents in grades (7, 8, 9, 10, 11, or 12) must complete, sign & upload the FHSAA Recruiting Policy Affidavit (GA4). The FHSAA GA4  is provided in the link above as the title of the form.
  • Please note all Shelton Academy-FHSAA athletic eligibility requirements must be submitted through your personal account created on athleticclearance.com and processed before attendance to the first day of tryouts and/or practice for interscholastic participation.
